There are more than just male and female. Only it's from an abnormality of chromosomes. Not from feeling uncomfortable with your gender. The feeling uncomfortable with your gender stuff you can blame on gender stereotypes, other societal pressures.

I really hate this issue because the real sufferers of the abnormality are lumped in with the rest. One has a social construct disease like bulimia, the other was born with physical abnormalities. Now those that can't help or change those things are treated like it's all in their head.
